PERMIT#: M-1991-009 <br /> INSPECTOR'S INITIALS: THM <br /> INSPECTION DATE: June 24,2014 <br /> OBSERVATIONS <br /> This inspection conducted as part of the routine inspection program established by the Division of Reclamation,Mining <br /> and Safety. The White River S&G Pit is a 6.7 acre 110c gravel pit located approximately 1.5 miles east of Meeker <br /> Colorado. Jack Paulson of White River Aggrgates, Inc. was present during the inspection. <br /> A mine identification sign was posted at the entrance of the site but was not in compliance with the requirements of Rule <br /> 3.1.12. Rule 3.1.12 requires the operator to post a staement indicating that a reclamation pemit for the operation has been <br /> issued by the Colorado Mined Land Reclamation Board along with the permit number. This is seen as a problem and the <br /> operator must provide photo-documention that demonstrates that the sign is in compliance with Rule 3 1 12 no later than <br /> August 8,2014 to avoid enforcment action. <br /> The 2014 annual report indicated that 9.99 acres was disturbed. Due to the permit consisting of only 6.7 acres the site was <br /> surveyed and DRMS staff found that the disturbance was slightly less than the approved 6.7 acres. <br /> The operation was active during the inspection but no mining activity was taking place. The activity consisted of <br /> construction material removal. <br /> The eastern extent of the permit had been regraded and vegetaed in accordance with the approved reclamation plan.The <br /> eastern end of the high wall located along the northern permit boundary was partially backfilled. <br /> Due to the site not being inspected since June 30, 2014 a reclamation cost update was conducted and will be sent under <br /> separate cover. <br /> Photos that show the conditions observed during the inspection are included at the end of this inspection report. <br /> If you have any questions regarding the conditions observed and documented in this inspection report please contact me at <br /> (970)241-2042. <br /> Page 2 of 4 <br />
