It <br /> has been revised to correct the acreages,to show the modification by the adoption of Option B, <br /> fix the owners list and structures list where mistakes were also found. <br /> EXHIBIT G -Water Information(Rule 6.4.7): The second paragraph of Exhibit G states that <br /> five monitoring wells have been Installed:on site,,however they do not appear anywhere on <br /> Exhibit C as stated. Please provide a map showing the locations of these wells,and the <br /> associated monitoring well boring and well construction logs for the five new wells. LG <br /> Everist will also need to commit to making the monthly groundwater level data collected a <br /> part of the annual report submitted to DRIVIS for this site. <br /> Only 4 wells were used not the-5 originally planned. I have added the 4 monitoring,wells to. <br /> lap Exhibit-C-1 - Current Conditions Map and provided a new map ekhibit in Exhibit G <br /> called MAP EXHIBIT G-2 -MONITORING WELL LOCATIONS, The monitoring well <br /> bore construction logs are attached and a Typical Monitoring Well Installation Sketch section to <br /> slow the well construction. Even though only 2 months-of data is available,the ground water <br /> Monitoring data Collected for each piezometer is provided. LG Everist,will commit to making <br /> e monthly groundwater level data collection a part of the permit and provide the information as <br /> part of the annual report for this site. <br /> EXHIBIT L-Reclamation Costs(Rule 6.4.12): Recalculation of the reclamation bond will be <br /> required. Please adjust the bond calculations to include the following: 100%of the cost of <br /> the western slurry wall must be submitted. DRMS'has-not yet been provided'with any <br /> detailed plans or design specifications for that slurry wall to-date.To qualify for the 20% <br /> bonding option, in addition to providing adequate plans and design specifications to DRMS <br /> for approval,the permittee will also need to provide construction QA/QC data to support <br /> that the slurry wall was installed In full compliance with the design specifications,-or pass <br /> the SEO testing criteria. So you are aware,the 20%bonding option "policy"for slurry walls, <br /> is currently under review by DRMS and may not be an available option In the future, <br /> I iave recalculated the Reclamation Cost estimate using your suggestion of including the <br /> I estern slurry wall at full cost since the detailed competition plans have not been submitted. <br /> L E may choose to supply the QA/QC and design documents at a later date and request a <br /> r duction to the 20% level should the,Division still be accepting that option. <br /> The existing exposed groundwater lake in the eastern area must be bonded for in <br /> some fashion. Bond may be calculated for the installation of a slurry wall to isolate <br /> It from the surrounding alluvial aquifer(see paragraph above for slurry wall <br /> calculation). Bond'for that area may also be determined using a defensible <br /> determination of cost for the placement of the amount of fill required to backfill the <br /> existing exposed groundwater area to no more than 6.1 acres of exposed <br /> groundwater,with the rest of the area backfilled to a Level at least two feet above <br /> the existing documented static water level.The backfilled areas must then be <br /> appropriately sloped and seeded.Areas to be re-sloped, otherwise re-graded, re- <br /> soiled,and reseeded should be shown on the provided reclamation plan map. <br /> en LGE opted to use Option B,I recalculated the bond to include backfilling the post- 1981 <br /> lace area to a least 2 feet above the ground water table.
