Permit M1990-057 3 <br /> Leadville Mill,Union Milling Company,LLC <br /> June 16,2014 <br /> 9. Weed control measures were performed as required. <br /> 10. The following permitting activities were performed during the year. <br /> • Lake County Conditional Use Permit review was held on November 4, 2013. The <br /> county commissioners unanimously approved the CUP in the public meeting. <br /> • An amendment to M1990-057 to convert from a 110(2) to a 110(d) was approved <br /> by CDRMS on June 11, 2014. Permit will become active as soon as reclamation <br /> bond amount is increased to account for the additional disturbance associated <br /> with the TSF upgrade and other requirements as dictated in our new 110(d) <br /> permit. <br /> No permanent reclamation occurred at the mill in the last 12 month period. Additional interim <br /> stabilization activities continued within existing disturbed areas. <br /> The bond for both the Leadville Mill and Hopemore Shaft totaling $81,700 remained in place. <br /> The $81,700 amount was determined by the Division in November 2007. The Leadville Mill <br /> reclamation estimate is $51,700 and the remaining $30,000 is for the Hopemore Shaft. An <br /> Irrevocable Letter of Credit remains in place for $51,700 from the Bank of Colorado of Yuma, <br /> CO for the Leadville Mill bond. <br /> Anticipated Activities: June 18, 2014 to June 17, 2015 <br /> No new disturbances are planned for the Leadville Mill. Construction activities will continue <br /> within the existing disturbed area to finish the tailings storage facility. Figure 6-2 is submitted <br /> showing the current disturbance outline and major proposed activities within the current <br /> disturbance area. <br /> UMC's activities for the year will occur in two phases; Phase 1 associated with environmental <br /> and permit-related activities, and Phase 2 work related with construction activities required to <br /> initiate mill operations. <br /> Phase 1 — Environmental & Permit Activities: <br /> 1. Update Reclamation Bond. $12,730 of additional bond is required to address additional <br /> disturbance associated with the expansion of the TSF as well as post-operational land <br /> use as described in our 110(d) permit. <br /> 2. Install new up- and down-gradient water monitoring wells. Our current monitoring well is <br /> approximately 22ft deep and is in a perched aquifer which has little, if any, water. UMC <br /> will drill 2 new monitoring wells as described in our new permit into the existing aquifer. <br /> 3. Pave Access Road at US Highway 24. As required by CDOT, UMC will pave the first <br /> 40ft of our access road from US highway 24. <br /> 4. Continue routine care and maintenance activities as required by our 110(d) and Lake <br /> County CUP permits <br /> Phase 2 —Construction Activities: <br /> Construction activities are dependent on our ability to raise the capital required to complete and <br /> operate the mill. Depressed precious metal prices have made it difficult for UMC to raise capital <br /> over the last few years, thus our delay in commissioning our plant. We are hopeful that we will <br /> secure funding this year. The following activities are will occur once funds are in place. <br /> 1 Mill equipment, replacement and repair summer 2014. <br /> 2 SWMP application July 2014. (No NPDES) <br /> 3 MSHA compliance anticipated July 2014. <br />
