RECEIVED <br /> June 12, 2014 `10N 16 2014 <br /> DIVISION OF RECLAMATIpN <br /> MINING AND SAFETY <br /> �Mr. Tyler O'Donnell AGGREGATE <br /> Division of Reclamation Mining and Safety <br /> INDUSTRIES <br /> 1313 Sherman Street, Room 215 <br /> Denver, CO 80203 <br /> Re: Permit Number M 2004-031 —Hazeltine Mine <br /> Technical Revision TR-3y <br /> Dear Mr. O'Donnell: <br /> Aggregate Industries(Al) submits this letter and enclosures in response to the Division's <br /> May 19, 2014 adequacy review comments and questions relative to the above-referenced <br /> technical revision regarding improvements, monitoring and maintenance of the Agridrain <br /> installed by Al in August 2013 to reduce surface water levels upgradient of the site. AI's <br /> responses are listed in the same order as set forth in the Division's adequacy letter. <br /> 1. Al commits to reporting the results of the Agridrain monitoring and maintenance <br /> plan to the Division each month. The enclosed form has been prepared for <br /> recording of weekly staff gauge observations and maintenance activities. The <br /> form has been set up to calculate the water elevation and includes prompt notes <br /> for types off conditions and maintenance activities to perform and record. <br /> 2. The staff gauge has been installed(see enclosed photos) and the elevation has <br /> been surveyed. Surface water elevation will be reported in feet above mean sea <br /> level (see enclosed form as referenced above). <br /> 3. Any maintenance performed during weekly monitoring events will be reported to <br /> the Division(see enclosed form as referenced above). <br /> 4. Al will outsource the weekly Agridrain monitoring and maintenance and has <br /> requested a cost proposal from a contractor to perform these activities. Upon AI's <br /> receipt of the proposal,the contractor will be asked to proceed immediately and to <br /> record and submit to Al a record of its activities each month. Al anticipates <br /> commencement of the weekly monitoring and maintenance by the end of June and <br /> proposes that the first monthly submittal to the Division will occur in August with <br /> a reporting of July's results. <br /> 5.
