PERMIT#:M-1994-118 <br /> INSPECTOR'S INITIALS: TCI <br /> INSPECTION DATE: September 3,2013 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the regular monitoring program. The Operator (Fremont County) was <br /> represented by Curtis Stone who was present for the inspection. This is a 110c gravel mine. The site was not <br /> active at the time of the inspection. <br /> The Phantom Canyon Pit is accessed from the Phantom Canyon Road (Co Rd 86) approximately five miles <br /> south of Victor, in Teller County. <br /> Inspection: <br /> Permit Boundary: A permit sign was displayed at the site entrance (see Photo 1). The site is fenced along the <br /> county road (west side), but not gated. The north and east sides are marked by the safety berm (see Photo 2). <br /> The working face of the highwall is on the south end of the disturbed area. T-posts were observed in the <br /> undisturbed southern area of the permit marking the southern boundary(see Photos 3 and 4). <br /> Pit & Permit Area: A small stockpile was observed in the southeast corner of the pit floor (Photo 5). Mining <br /> activity was confined to the north end of the permit area and is advancing to the south. No standing water was <br /> observed in either pit. <br /> Reclamation/Reve etg ation: No reclamation had been performed at the time of the inspection. The Operator has <br /> not initiated any revegetation. <br /> Current Stipulations: <br /> There is one stipulation requiring the Operator to adhere to the reclamation plan provided by the Soil <br /> Conservation Service included in the 1994 permit application. The Division determined this stipulation is no <br /> longer necessary as the SCS plan is part of the permit and the Operator committed to following the approved <br /> reclamation plan as part of the AM-01 application. <br /> Records: <br /> • The previous inspection was conducted 8/4/2009. No problems were cited. <br /> • The permit anniversary date is August 8. The Operator is current with annual fees and reports. <br /> • There have been three requested revisions to this permit: <br /> o TR-01 was withdrawn on 12/2/2003; <br /> o AM-01 and AR-01 facilitated a land swap to change the southern boundary and were <br /> approved at the same time on 12/26/2003. <br /> • The 2013 annual report map does not reflect the modifications made to the boundary in the above <br /> revisions. The approved mine plan map from AM-01 is attached for reference. <br /> • The pre-mine land use is wildlife habitat and the post-mine land use is recreation. <br /> Bond: <br /> As this is a county pit, no bond is required. <br /> Summary and Recommendations: <br /> 1. The mine appears to be operating in accordance with the approved permit and subsequent revisions. <br /> 2. Please use a current map that reflects the AM-01 approved boundary for future annual reports. <br /> Page 2 of 5 <br />
