PERMIT#:M-1995-008 <br /> INSPECTOR'S INITIALS:TC1 <br /> INSPECTION DATE: May 20,2014 <br /> OBSERVATIONS <br /> This inspection was initiated by a request for Temporary Cessation. The purpose of this inspection was to check <br /> compliance of the operation with the requirements of the Act, Rules, and the conditions of the permit. Jordan <br /> Sasser and John Young were present during the inspection representing the Operator. The site entrance is <br /> located approximately one half mile north of Florence, on the east side of Highway 67, in Fremont County. <br /> This is 112c sand and gravel mining operation, also referred by the Operator as the LA Methodist Pit. The site <br /> was not active at the time of the inspection. <br /> Inspection: <br /> Permit Boundary: A permit sign was posted on the entrance road (see Photo 1). Pit boundary signs were <br /> observed wired to the barbed wire phase 2 boundary fence (see Photo 2). <br /> Pit & Permit Area: No stockpiles were observed in either phase 1 or phase 2 of the pit area. The most recent <br /> activity took place in phase 2, north of the gas line. Ms. Sasser and Mr. Young indicated slope regrading in the <br /> phase 2 area (Photos 3 - 4) occurred recently. A small amount of recontouring remains to be completed (Photo <br /> 5). Ms. Sasser stated the Operator will seed the recontoured area once grading is complete. <br /> Mining,Activity: Neither Ms. Sasser or Mr. Young had an estimate for when the last mining activity took place. <br /> A review of Google Earth historical images indicates additional stockpiles were being removed as late as 2006. <br /> Reclamation: No formal reclamation has been performed to date (referencing the 2014 annual report). The <br /> phase 1 portion of the mine, south of the gas line appears to be largely self-reclaimed (Photos 6 - 7). The <br /> Division recommended the Operator request an Acreage Release for phase 1. <br /> No standing water was observed in the pit. Ms. Sasser indicated the site Stormwater Management Plan is kept <br /> on file in the Canon City office. Some erosion gullying was observed in the north end of the phase 2 area (see <br /> Photo 8). This area should be regraded and BMPs implemented. <br /> Current Stipulations: <br /> None. <br /> Records: <br /> • The previous inspection was conducted 6/10/2009. No problems were cited. <br /> • The permit anniversary date is April 18. The Operator is current with annual fees and reports. <br /> • The 2014 annual report indicates 4 to 5 acres of disturbance. The Division concurs that the phase 2 area <br /> of disturbance encompasses 4 to 5 acres. <br /> • The only revision(TR-01) for this site has been requested. The Division approved a seed mix change on <br /> February 9, 2000. <br /> • The pre- and post-mine land use is rangeland. <br /> Bond: <br /> The current financial warranty is a $33,700.00 corporate surety. The bond is adequate for the remaining <br /> grading and seeding required for phase 2. <br /> Summary and Recommendations: <br /> 1. The Division will recommend to the Mined Land Reclamation Board (MLRB) the site be approved for <br /> Temporary Cessation (TC). The Operator will be notified of the Board's approval under separate cover. <br /> Page 2 of 7 <br />
