PERMIT #: M- 1977 -342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: April 29, 2014 <br />OBSERVATIONS <br />The Henderson Mine was inspected by Peter Hays, Amy Eschberger and Elliott Russell with the Division of <br />Reclamation, Mining and Safety (Division) as part of the Division's monitoring inspection program. Mr. Bryce <br />Romig, Mr. Miguel Hamarat, Mr. Tim Haynes and Ms. Amber Moran with Climax Molybdenum - Henderson <br />Operations (Henderson) were present during the inspection. <br />A tour of the mill facilities was conducted as part of the inspection. The following facilities were observed <br />during the inspection; the PC 2 / PC 3 transfer station, west portal, tailings impoundment, tailings delivery <br />lines and the mill building. <br />The inspection concluded with a meeting to discuss the ongoing permitting items at the site. The following <br />topics were discussed: financial warranty structure, tailings delivery line structural support refurbishment, <br />removal of Drop Towers 15 and 16, the ongoing pH investigations for MNGW -1 and MLGW -7 and the inclusion <br />of the Urad Water Treatment Facility into the Henderson permit. <br />Financial Warranty: <br />Henderson is waiting for the Colorado Mined Land Reclamation Board (Board) to make a determination on the <br />financial warranty structure proposed for the Climax Mine prior to submitting the revised bond estimate for <br />the site. The Henderson site will request a similar financial warranty if the Climax warranty structure is <br />approved. The item is expected to be presented before the Board during the July 30, 2014 meeting. <br />Henderson requested clarification on whether the Division will require indirect costs to be added to the water <br />treatment portion of the current bond calculation. The Division will discuss the item with senior staff and <br />respond to Henderson under separate correspondence. <br />Tailing Delivery Line (TDL) structural support refurbishment and removal of Drop Towers 15 and 16• <br />The removal of Drop Towers 15 and 16, the replacement of 297 linear feet of RCP with 42 -inch HDPE pipe and <br />structural support refurbishment of the Tailing Delivery Line (TDL) was approved by the Division March 27, <br />2014 under TR -22. The project is scheduled to begin in June 2014 and take 3 months to complete. <br />MNGW -1 and MLGW -7: <br />Henderson submitted a copy of the Mill Groundwater Quality Assessment for MLGW -7 Low pH report by Ajax <br />and Clear Creek Associates dated March 20, 2014 to the Division for review. Henderson plans to submit a <br />technical revision (TR -23) with additional groundwater data and information regarding Phase 1 of the new <br />extraction well field in the near future. <br />The plans to install two additional groundwater monitoring wells adjacent to MNGW -1 to determine the <br />potential influence of the adjacent development rock and No Name Creek on the POC well were delayed until <br />this spring. The installation of the wells is now planned for July or August 2014, following the issuance of the <br />necessary wetland permits. Road and infrastructure work to access the area will begin soon. <br />Additional groundwater items: Henderson stated 5 quarters of groundwater data for the monitoring wells <br />installed as part of the groundwater monitoring plan (TR -16) would be submitted to the Division for review. <br />The new data indicates a POC well is not needed in Potato Gulch and a POC well is needed below 3 -Dam <br />according to Henderson staff. <br />Page 2 of 3 <br />
