OTHER PERMITS AND LICENSES REQUIRED EXHIBIT M <br />The following permits are needed for the site: <br />1) County Special Use — The original permit was issued by Mesa County on February 11, 2003. <br />The revised application for the additional lands in the Amendment of 2006 was also approved by <br />Mesa County. <br />2) NPDES combined process water /storm water discharge permit — Permit #COG - 500437 has <br />been issued for this site. <br />3) Air Emissions — A fugitive dust permit has been issued by the CDPHE for the site (Permit # <br />02ME0988F). Additional permits will be obtained for all portable plants which are brought to the <br />site. <br />4) Gravel Well Permit — Permit number 62210 -F has been issued for the amended area on the <br />Mesa Grand, LLC property. United obtained a gravel well permit for the original permit area in <br />late 2002 (Permit 58799 -F), but it will need to be modified to account for a larger water surface <br />area for evaporation. This is being submitted to the Water Resources Division. <br />5) A State Highway Access Permit has been issued for vehicles entering /leaving Highway 50. <br />6) A Spill and Containment plan has been made for the site since fuel storage on site will occur. <br />This is available on site. <br />7) An individual COE permit was issued to disturb the minor wetlands on the United property for <br />the initial permit in 2003. The COE also evaluated the data from Westwater Consultants to <br />determine the requirements for the small amount of wetlands on the Mesa Grand, LLC additional <br />area for the 2006 Amendment.
