PERMIT #: M- 1990 -098 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: April 14, 2014 <br />technical revision and should address the problem cited during the October 28, 2002 inspection. A copy of the <br />404 permit will be submitted to the Division by NCCI following the permit issuance. <br />Overburden and topsoil stockpiles were observed in the northeast corner of the site along the north and east <br />permit boundaries. A stockpile of rip rap was also observed in the northeast corner of the site. A large <br />stockpile of processed material was observed in the middle of the site southwest of the scale house. The <br />dewatering pump in Pond C, processing plant and equipment were also observed during the inspection. <br />A slurry wall is installed around the perimeter of the site. The slurry wall was approved by the Division of <br />Water Resources in 2006. The previous operator did not request a financial warranty reduction following the <br />liner approval. Two sections of the slurry wall, along the west and south sides of the site, were breached <br />during the flood event last year. The slurry wall will be repaired by the City of Fort Lupton, which owns the <br />reservoir in the near future. NCCI plans to submit a financial warranty reduction request to reduce the <br />required bond for the completed portion of the slurry wall. <br />Updated reclamation maps indicating three spillways are available for the site. NCCI will submit the revised <br />maps to the Division as a technical revision in the future. <br />Photographs taken during the inspection are attached. <br />Page 3 of 6 <br />
