As discussed during our phone conversation, Grand Gravel will be required to submit an amendment application to the current conversion application <br />due to the survey error. <br />A new permit application form and all exhibits changed by the survey error will need to be submitted to the Division as part of the amendment <br />application. All notices, newspaper publication and landowner and structure owner notices, must be re- noticed as part of the amendment process. A <br />new decision date for the conversion and amendment applications will be set for 90 days after the amendment application is complete and considered <br />filed by the Division. <br />The amendment permit application form is available at http: // mining. state. co. us/ SiteColIectionDocuments /Construction112App.pdf <br />No fee is required for the amendment, since the conversion application is still open. <br />Please contact me with questions. <br />Peter S.
