DRMS 04/19: At the bottom of page 5 -3, please direct the reader to the portion of the <br />permit where the approved designs for the Carbon Junction Canyon permanent diversion <br />may be found. <br />DRMS 12/18: The text was not added, as requested. However, in partial abatement of <br />Notice of Violation CV- 2013 -005, as -built plans for the permanent diversion were <br />incorporated into the permit with the approval of TR -17 in Oct -2013. Please add a <br />reference in this section to new Appendix 5 -12. <br />DRMS 04125: Text added with the Mar -2014 response refers to modifications being <br />approved; the text should show instead that As -Built plans for the permanent diversion <br />(this is the riprap -lined channel that skirts the PRSA) are found in Appendix 5 -12. Please <br />revise the APD text accordingly. <br />DRMS 12/18: The last paragraph of subsection 2.053(3) addresses roads that will be <br />retained as permanent features. Please expand this list to include the Light Use Road <br />built to access well B 10. (A letter from Oakridge, requesting retention of this road, was <br />provided with the adequacy response.) <br />DRMS 04/24: The list of roads was revised, as requested. This item is resolved. <br />4. DRMS 12118: Designs for the reestablishment of the Upper Carbon Junction Channel <br />were approved under TR -17 on 29- Oct -2013. The work was accomplished during <br />November 2013, completing abatement of CV- 2013 -005. Please update the Stream <br />Channel Reconstruction section of 2.05.3(4) to include this work and a reference to <br />Appendix 5 -12, where the designs may be found. <br />DRMS 04/25: Text reflecting the reestablishment of the Upper Carbon Junction Canyon <br />under TR -17 was added. However, the paragraph regarding the "permanent diversion" <br />should be deleted — the permanent diversion is the riprap -lined channel that skirts the <br />PRSA. Please delete this paragraph from the Reconstruction section of the ADP. <br />2.05 4 — Reclamation Plan <br />a) DRMS 12/18: The Mine Openings subsection of the APD (page 5 -15) was expanded <br />to address the current status of the five monitoring or water supply wells. Please provide <br />a reference to direct the reader to the portion of the original permit where the drill logs <br />for the five wells may be found. [Drill logs for l OB (B 10), 17B (B 17), and 82 -4 were <br />found in Appendix 4 -3 of the permit; the Division was unable to find drill logs for 94 -1 <br />and 95 -1.] With respect to sealing of wells B10, B17, and 94 -1, information provided in <br />the Aug -2013 response is incomplete. The Division suggests that Oakridge might contact <br />the reclamation contractor(s) to obtain the details required by Rule 4.07.3(3)(f) and (g) <br />for inclusion in the APD. <br />DRMS 04/25: References to Appendix 4 -3 for the first three wells were not inserted. <br />The reference to 94 -1 and 95 -1 having been contained in the original Carbon Junction <br />
