PERMIT #: M- 2006 -081 <br />INSPECTOR'S INITIALS: TCI <br />INSPECTION DATE: July 25, 2013 <br />OBSERVATIONS <br />This inspection was initiated by a request for Temporary Cessation. The purpose of this inspection was to check <br />compliance of the operation with the requirements of the Act, Rules, and the conditions of the permit. Dan <br />Gaudreault was present during the inspection representing the Operator. This site is located approximately 24 <br />miles east of Pueblo, in Pueblo County. The site is north of US Highway 50 near 61" Lane. <br />Inspection: <br />The Permit sign and boundary markers were properly posted (Photo 1). The mine was not active at the time of <br />the inspection. Several stockpiles of various sized materials were observed There has been no activity on site <br />since the previous inspection other than some asphalt recycling near the entrance (Photos 2 - 5), as well as <br />vegetative media stockpile; offset to the south (Photo 6). <br />According to the April 2013 annual report, no mining activity has occurred since February 20, 2011. As the <br />Division formalized the temporary cessation procedures in 2013, we will accept the June 12, 2013 date <br />requested as the beginning for the initial 5 -year period of temporary cessation. <br />Current Stipulations: <br />None. <br />Records: <br />• The previous inspection was conducted 9/23/2011. No problems were cited. <br />• The permit anniversary date is April 24. The operator is current with annual fees and reports through <br />2013. <br />Bond: <br />The current financial warranty is a $43,303.00 letter of credit and is deemed adequate based on the approximate <br />current approximate two -acre disturbance. <br />Summary and Recommendations: <br />1. Based on the inspection, the Division recommended and approved the request for Temporary Cessation <br />on August 14, 2013. <br />Page 2 of 4 <br />
