Drill hole SR -22A: In rechecking the drill log data for SR -22A, the drill log's data does not <br />indicate if this hole was sealed or not sealed. Assuming that the Boreholes & Wells — <br />Reclamation Status table (the Table) is in error, the table for SR -22A has been changed from <br />"sealed" to "open" with a question mark (Page 1). The status of the borehole can be determined <br />by the required field reconnaissance. <br />2. SR -55 and SR -56: The drill log data for SR -55 & SR -56 does not indicate if these holes were <br />sealed or not. If plugging affidavits existed in 1983 as indicated by Mr. McEvers letter (1983), <br />apparently the records were not transferred to Energy Fuels at the time of purchase of the <br />Southfield mine. Accordingly, the Table has been changed (Page 3) from sealed to N/D (no <br />data). A further letter from Mr. McEvers dated 1 -27 -1983 indicates that SR -55 & SR -56 were <br />reclaimed and reseeded on December 18 -19, 1982. Additional records can not be found. The <br />status of the boreholes can be determined by the required field reconnaissance. <br />3. Abandonment Reports — SF series: Any borehole abandonment reports that were available <br />have previously been submitted to the Division. As EFCI has previously stated, an extensive and <br />thorough records search for abandonment reports was completed in 2007 and again in 2013 - - no <br />additional reports are available. EFCI has exhausted the records searches. The status of the <br />boreholes can be determined by the required field reconnaissance. <br />The Division requests an explanation as to why the respective reports are missing. Energy Fuels' <br />interrelated company's central files have been relocated twice during the past 30 years. If <br />respective abandonment reports existed, the logical conclusion is that they were lost or misplaced <br />during the relocation of the archives during the past 30 years. <br />
