Mr. West <br />Page 14 <br />April 1, 2014 <br />11. As required by Rule 6.3.5(2)(d), the Mining Plan Map must indicate the direction that <br />mineral extraction will proceed. <br />12. The applicant has submitted a Reclamation Plan Map which depicts the currently permitted <br />3.39 acres. Please revise the Reclamation Plan Map to depict the proposed 9 acre permit area. <br />In addition, the Reclamation Plan Map must include the following: <br />a) Show the name of the Applicant. <br />b) Must be prepared and signed by a registered land surveyor, professional engineer, or <br />other qualified person. <br />c) Give the date the map was prepared. <br />d) Identify and outline the area that corresponds with the application. <br />e) Include a north arrow, map title, appropriate legend and a map scale. The acceptable <br />range of map scales shall not be larger than 1 inch = 50 feet nor smaller than 1 inch = 660 <br />feet. <br />14. As required by Rule 6.3.5(3)(a), the Reclamation Plan Map must show the gradient of all <br />reclaimed slopes sufficient to describe the post mine topography. <br />6.3.6 Exhibit F — List of Other Permits and Licenses Required <br />15. The applicant has a currently approved Plan of Operations (POO) with the U.S. Forest <br />Service (USFS). However, a new POO must be obtained from the USFS to account for any <br />changes to the mining and reclamation plan which result from the approval of the <br />Amendment Application. Please revise Exhibit F to include a statement that a new POO will <br />be obtained. In addition, please commit to providing the Division with a copy of the <br />approved POO. <br />6.3.7 Exhibit G — Source of Legal Right to Enter <br />No comment. <br />6.3.8 Exhibit H — Municipalities Within a Two Mile Radius <br />No comment. <br />6.3.9 Exhibit I — Proof of Filing with County Clerk <br />No comment. <br />6.3.10 Exhibit J — Proof of Mailing of Notices to Board of County Commissioners and Soil <br />Conservation District <br />No comment. <br />
