Please submit a revised Exhibit A with updated <br />coordinates for the proposed 9 acre permit boundary or remove the latitude /longitude <br />coordinates from this exhibit as they are not required by rule 6.3.1. <br />6.3.2 Exhibit B — Site Description <br />3. The Applicant has provided the latitude /longitude coordinates for the proposed permitted <br />area. The coordinates do not define the proposed affected area of 9 acres, but rather define <br />the already permitted area of 3.39 acres. Please submit a revised Exhibit B with updated <br />coordinates for the proposed 9 acre permit boundary or remove the latitude /longitude <br />coordinates from this exhibit as they are not required by rule 6.3.2. <br />6.3.3 Exhibit C — Mining Plan <br />4. The Mining Plan states there are two piles of mixed ore, overburden and topsoil at the mine <br />site. The currently approved Mining Plan specifies that ore, overburden and topsoil will be <br />stockpiled separately. Please confirm that all existing stockpiles have been classified and <br />revise the description of the existing stockpiles in the Mining Plan. In addition, please specify <br />the volume of all existing stockpiles. <br />5. The Mining Plan states that the existing access road is included within the affected area and <br />will be reclaimed when the mining operation is complete. The Applicant has stated that the <br />access road is 150 feet in length. Exhibit E(1), specifies the coordinates for the intersection of <br />the access road with Forest Service Road 226. Using the specified coordinates and an aerial <br />photograph of the mine site, the Division has determined that the distance from Forest <br />Service Road 226 to the northern permit boundary is approximately 240 feet. Please confirm <br />that the access road described in the Mining Plan connects the north mine entrance to Forest <br />Road 226. Also, please revise the Mining Plan to include accurate dimensions (length and <br />width) of the access road. <br />6. The Mining Plan states ore will be removed and shipped off site. The Division observed ore <br />stockpiles at the site during inspections conducted on August 7, 2013 and October 23, 2013. <br />Please revise the mining plan to include a description of how ore will be handled once it has <br />been extracted. The Mining Plan must specify how much ore will be stockpiled at any given <br />time, the location of the ore stockpile and a description of how the ore will be handled so that <br />the drainage system is protected from pollution and unsightliness is minimized. <br />6.3.4 Exhibit D — Reclamation Plan <br />7. The Reclamation Plan and the approved Plan of Operations state that concurrent reclamation <br />will occur, including backfilling of all pits and replacement of topsoil at the end of each <br />season. The Division observed overburden and topsoil stockpiles at the site during <br />inspections conducted on August 7, 2013 and October 23, 2013. In addition, the open pit was <br />not backfilled at the end of the 2013 mining season. The Operator must provide a detailed <br />plan for how the major disturbance at the site will be reclaimed at the end of each mining <br />
