Randy Schafer <br />April 4, 2014 <br />Page 2 of 3 <br />6.4.3 Exhibit C - Pre - Mining and Mining Plan Maps of Affected Land <br />4. Please provide an updated map showing the actual location of the Logan County Kokes Pit (M -1978- <br />249) boundaries as compared to the proposed expansion boundaries of the MMM Partnership Gravel <br />Pit. Enclosed is a copy of the survey plat for the approved Kokes Pit boundaries. This was submitted <br />on 10/25/2001 and can be found on the Division's image document database at the following website: <br />http://drmsweblink.state.co.us/drmsweblink/search.aspx?dbid=0. <br />5. As required by Rule 6.2.1, all maps must be prepared and signed by a registered land surveyor, <br />professional engineer, or other qualified person. Please sign the Exhibit C maps and all other maps <br />submitted for the conversion application. <br />6.4.4 Exhibit D - Mining Plan <br />6. Please provide a signed and notarized letter from the landowner stating the access road into the site <br />was pre- existing and shall remain after final reclamation of the mine complete. Please note that this <br />road is allowed to be maintained, but if the road is widened to accommodate two -way traffic an <br />amendment to the permit must be submitted to the Division to account for the increase in disturbance <br />area. <br />7. Please state the depth and thickness of the deposit to be mined as required by Rule 6.4.4(f)(i). <br />8. Please state the nature of the stratum immediately beneath the material to be mined as required by <br />Rule 6.4.4(f)(ii). <br />6.4.5 Exhibit E — Reclamation Plan <br />9. To be acceptable to the Division, please replace the following wording in the reclamation plan <br />narrative from "any slopes created will be graded 3h:Iv more or less" with "any slopes created will be <br />graded no steeper than 3h:Iv, unless approved by the Division ". <br />10. Please clarify the number and location of topsoil stockpiles and update the reclamation plan narrative <br />and mining plan map as necessary. <br />11. Please specify the tons /acre of straw that will be used to mulch the reclaimed area. <br />12. As required by Rule 6.2.1, all maps must be prepared and signed by a registered land surveyor, <br />professional engineer, or other qualified person. Please sign Exhibit E and all other maps submitted <br />for the conversion application. <br />13. The Applicant stated that fencing may be needed during the first two years after seeding to prevent <br />livestock from grazing on the reclaimed area. Please state that this fence will be taken down or will be <br />left for the landowner. If the latter is chosen, please submit a signed and notarized letter from the <br />landowner allowing the fence to remain after final reclamation is complete. <br />6.4.12 Exhibit L - Reclamation Costs <br />14. The Division will estimate the cost to reclaim the site based on the information submitted once the <br />Applicant addresses the concerns noted in this letter. <br />
