Director <br />Dba United Companies of Mesa County <br />2273 River Road <br />Grand Junction, CO 81502 <br />Re: Mamm Creek Sand and Gravel Pit, Permit M- 2000 -113, Succession of Operators Approval, Revision SO -2, <br />Stipulation Complied With <br />Dear Jason Burkey: <br />On February 24, 2014, the Division of Reclamation, Mining and Safety approved with the following stipulation, the succession of <br />operators from Lafarge West, Inc. to Oldcastle SW Group, Inc. dba United Companies of Mesa County for this operation. Oldcastle SW <br />Group, Inc.dba United Companies of Mesa County is now the permitted operator of the Mamm Creek Sand and Gravel Pit, and as such, <br />is responsible for all provisions in M- 2000 -113, as well as those specified in the Rules and Regulations. Lafarge West, Inc. is relieved of <br />all responsibilities concerning this operation. <br />Stipulation No. 2 <br />Oldcastle SW Group, Inc. dba United Companies of Mesa County to submit all required properly executed structure damage waiver <br />agreements within 180 days of this approval of Succession of Operator. If waivers cannot be obtained within 180 days, the operator shall <br />submit a geotechnical evaluation including engineering stability analysis as required per CRS 34- 32.5- 116(4) (i) and Rule 6.5. <br />On April 1, 2014 the Division received the properly executed Structure Damage Waiver Agreement from the landowner James G. <br />Snyder.
