PERMIT #: M -1998 -013 <br />INSPECTOR'S INITIALS: ECS <br />INSPECTION DATE: April 2, 2014 <br />OBSERVATIONS <br />This was a Pre - Operation inspection conducted for Amendment AM02 by DRMS at the Bestway Concrete "The <br />Shores" site. The inspection was conducted by Eric Scott of DRMS. The DRMS representative traveled to the <br />site and met with permittee representative Mark Johnson of Bestway Concrete at approximately 0930. There <br />are 309.8 acres currently permitted at this 112c site. AM02 will increase the permitted acreage by <br />approximately 60 acres. DRMS noted that the permit boundary markers for the northern amendment area <br />(currently farmland as shown in attached photos) were missing along portions of the south and west sides. <br />These markers will need to be put in place before the corrective action date listed on the first page of this <br />report, and before amendment is approved. <br />There is also an area on the southeast corner of the existing permit currently used for stockpiling overburden <br />and topsoil. To avoid enforcement action for mining activity impacts outside the permitted area; this area will <br />also need to be included into the pending amendment even though no excavation is proposed for that area. <br />All maps /exhibits, affected area totals, and text will need to be revised as appropriate throughout the AM02 <br />submittal, and revised copies of those changes will need to be placed in the copy for public review filed with <br />the County. The permit boundary will also need to be clearly marked to define the stockpile area. Other than <br />these boundary issues, the mining activity was taking place according to the approved mining plan at the time <br />of the inspection. <br />The required additional groundwater level monitoring well locations, well construction logs, groundwater <br />monitoring plan, and specifics of the mining plan as it relates to the required bonding revision (slurry wall <br />installation /topsoil overburden stripping, re- vegetation, etc) for the amended areas will also need to be <br />provided to the Division for review and approval before the pending amendment can be approved. <br />Inspection Contact Address <br />Mark Johnson <br />Bestway Concrete Company <br />301 Centennial Dr. <br />Milliken, CO 80543 <br />Page 2 of 4 <br />
