Pineda <br />Director <br />On February 25, 2014, the Division of Reclamation, Mining and Safety approved with the following stipulation, the succession of <br />operators from Lafarge West, Inc. to Oldcastle SW Group, Inc. dba United Companies of Mesa County for this operation. Oldcastle SW <br />Group, Inc.dba United Companies of Mesa County is now the permitted operator of the Blair Mesa Pit, and as such, is responsible for all <br />provisions in M- 1996 -082, as well as those specified in the Rules and Regulations. Lafarge West, Inc. is relieved of all responsibilities <br />concerning this operation. <br />Stipulation No. 2 <br />Oldcastle SW Group, Inc. dba United Companies of Mesa County to submit all required properly executed structure damage waiver <br />agreements within 180 days of this approval of Succession of Operator. If waivers cannot be obtained within 180 days, the operator shall <br />submit a geotechnical evaluation including engineering stability analysis as required per CRS 34- 32.5 - 116(4)(1) and Rule 6.5. <br />On February 25, 2014 the Division received the properly executed Right to Enter and Structure Damage Waiver Agreement from the <br />landowners Carroll Davidson Partnership, Ltd. Oldcastle SW Group, Inc., dba United Companies of Mesa County has complied with the <br />Stipulation No. 2 and the Succession of Operator now has final approval. <br />Sincerely, <br />6*';v jg-- <br />Travis H.
