PERMIT #: M- 2002 -114 <br />INSPECTOR'S INITIALS: THM <br />INSPECTION DATE: March 12, 2014 <br />OBSERVATIONS <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division <br />of Reclamation, Mining and Safety (Division). The 15 Road Pit is located 3 miles west of Fruita, Colorado off <br />of Highway 6 & 50. The site is a 112c operation that is currently active. Jason Burkey representing Oldcastle <br />SW dba United Companies of Mesa County accompanied the inspector during the inspection. <br />The pit was active during the inspection. Mining activity was primarily taking place in Mining area 4 in Phases <br />1, 2 and 3. Large stockpiles of construction material were observed in Mining Area 4 along with various <br />material processing equipment. The mining sequence is following the approved mine plan with respect to the <br />order of mine phase sequence which was revised on April 8, 2011 with the approval of Technical Revision No. <br />4. <br />A vegetative test plot was observed in Pit 1. The purpose of the test plot is to determine if the <br />introduction/comingling of biosolids from the adjacent Fruita sewage treatment plant will have a positive <br />impact on revegetative efforts. The use of biosolids is not currently a component of the approved reclamation <br />plan. The current use of biosolids within the permit area is considered a problem and a Technical <br />Revision to the Reclamation Plan is due no later than April 24, 2014. <br />The currently approved mine plan allows for a total disturbance of 86 acres and according to the annual report <br />received on March 5, 2014 the total acres currently affected is 105. The current amount of affected acreage <br />is considered a problem and a Technical Revision to the Mining Plan is due no later than April 24, 2014. <br />Reclamation observed at Pit 2 appeared to be in compliance with the approved reclamation plan. Evidence of <br />various perennial grasses becoming established was observed. No noxious weeds were observed within the Pit <br />2 area. <br />No offsite damage or erosional issues were observed during the inspection. <br />If you need more information regarding this inspection report please contact Travis Marshall in the DRMS <br />Grand Junction Field Office at (970) 241 -2042. <br />Page 3 of 5 <br />
