PERMIT #: M- 1977 -215 <br />INSPECTOR'S INITIALS: RCO <br />INSPECTION DATE: February 24, 2014 <br />the pad. The safety and drainage berm is intact. The 9400 portal is locked since the permit is in temporary <br />cessation. There is no mobile equipment at the portal area. The 9360 pad and facilities area contains only <br />minimal equipment (one dozer) and a few light duty vehicles. The shotcrete building and shop are empty, <br />there are no explosives presently stored onsite, and diesel storage (inside the storage building) is minimal. <br />There are still some parts and materials stored onsite, presently less than when the site was active. These <br />include lengths of steel vent duct and steel sets for portal construction, other various steel parts, concrete <br />mix, and numerous 250 - gallon industrial plastic containers (that were empty). <br />The operator is maintaining and monitoring the site. The site appears stable and the requested status of <br />temporary cessation appears approvable. The decision for this requested status will be made by the Board, <br />during the upcoming March 2014 Board meeting at the earliest. The operator is preparing the EPP for <br />submittal to the Division in May 2014. The EPP will address topics of stormwater controls, hydrologic <br />monitoring, SPCC, which were discussed during this inspection. <br />At this time the financial warranty is considered adequate, since it was recalculated for the last revision in <br />2013.
