PERMIT #: M- 1985 -210 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: March 6, 2014 <br />1) A plan for sampling and removing all contaminated soil in the equipment service area. <br />2) An inventory of all types and volumes of liquids stored at the mine site. <br />3) A description of the secondary containment for each tank on site. <br />4) A description of how fluids will be managed, including storage and disposal of such fluids. <br />5) A description of how minor spills will be cleaned up. <br />6) A commitment to provide the Division with a final report with the appropriate maps, volume of soil <br />removed, sample locations, analytical data and photographic documentation. <br />7) A commitment to provide the Division with evidence in the form of a receipt that the contaminated soil was <br />disposed of by an approved method. <br />The Division will conduct a follow -up inspection once the Operator provides notification that all contaminated <br />soil has been removed. In addition to the corrective actions issued by the Division, the Operator must <br />complete any corrective actions issued by HMWMD. <br />This concluded the inspection. <br />Page 3 of 8 <br />
