COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT, Water Quality Control Division <br />Rationale - Page 15. Permit No. COR- 030000 <br />VIII. TERMS AND CONDITIONS OF PERMIT (cont.) <br />2. Transfer of Permit: When responsibility for stormwater discharges for an entire construction site <br />changes from one individual to another, the permit shall be transferred in accordance with Part I.A. 7 of <br />the permit. The permittee shall submit a completed Notice of Transfer form, which is available from <br />the Division, and at www.cdphe.state.co. us /wq/PermitsUnit. If the new responsible party will not <br />complete the transfer form, the permit may be inactivated if the permittee has no legal responsibility, <br />through ownership or contract, for the construction activities at the site. In this case, the new owner or <br />operator would be required to obtain permit coverage separately. <br />3. Reassignment of Permit: When a permittee no longer has control of a specific portion of a permitted <br />site, and wishes to transfer coverage of that portion of the site to a second party, the permittee shall <br />submit a completed Notice of Reassignment of Permit Coverage form, which is available from the <br />Division, and at www.cdphe.state.co.us/wq /Permits Unit. The form requires that both the existing <br />permittee and new permittee complete their respective sections. See Part 1A.8 of the permit. <br />J. Duration of Permit <br />The general permit will expire on June 30, 2012. The permittee's authority to discharge under this permit is <br />approved until the expiration date of the general permit. Any permittee desiring continued coverage under the <br />general permit past the expiration date must apply for recertification under the general permit at least 90 days <br />prior to its expiration date. <br />Kathleen Rosow <br />December 18, 2006 <br />IX. PUBLIC NOTICE — 12/22/06 <br />The permit was sent to public notice on December 22, 2006 A public meeting was requested, and was held on <br />February 2, 2007. Numerous comments were received on the draft permit. Responses to those comments, and a <br />summary of changes made to the draft permit, are in a separate document entitled "Division Response To Public <br />Comments. " The permit will be sent to a second public notice on March 23, 2007. Any changes resulting from the <br />second public notice will be summarized in the rationale. <br />Kathleen Rosow <br />March 22, 2007 <br />X. PUBLIC NOTICE — 3123107 <br />The permit was sent to public notice for a second time on March 23, 2007. Numerous comments were received on the <br />second draft permit. Responses to those comments, and a summary of the additional changes made to the draft <br />permit, are contained in a separate document entitled "Division Response To Public Comments Part II ". This <br />document is part of the rationale. Any changes based on the Division response are incorporated into the rationale and <br />permit.
