PERMIT #: M- 2010 -047 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: February 21, 2014 <br />A letter titled "Review of Current Permit Status Regarding Temporary Cessation" was sent on February 28, <br />2013 to the Operator explaining intermittent operation status and Temporary Cessation of mining operations. <br />A copy of the letter is attached. The 2013 Annual Report submitted by the Operator indicated the site was not <br />active for more than 180 days; therefore the Division recommends the Operator apply for Intermittent Status <br />(IS) by submitting a technical revision for the site. <br />As cited as a problem on the first page, the mine sign as required by Rule 3.1.12(1) was not observed during <br />the inspection. The Operator will be required to post a sign and provide photographic evidence to the Division <br />by the corrective action date. The permit boundary was defined by fence on all sides. <br />The Division currently holds a financial warranty in the amount of $51,100.00, which appeared adequate to <br />complete reclamation of the disturbed area observed during this inspection. <br />The approved Mine Plan requires the Operator to mark the 30 foot offset from the permit boundary, stockpile <br />topsoil along CR 37 north of the entrance, clean the culvert under CR 37 periodically and remove the debris in <br />the southern portion of the site. None of these requirements were observed during the inspection and should <br />be completed by the Operator. <br />The site was not active during the inspection, however a processing plant and stockpiled material were <br />observed on the pit floor. The eastern Mining Area 1 highwall was estimated at 10 to 12 in height and the <br />western highwall was estimated at 8 -10 feet in height. All mining activity appeared to be contained in Mining <br />Area 1. The current mining disturbance was GPS surveyed by the Division using a Trimble Juno 3B. A map of <br />the survey results is attached. The survey indicated a permit area of 39.56 acres with approximately 5.98 <br />acres of current mining disturbance. <br />Photographs taken during the inspection are attached. <br />Page 2 of 5 <br />
