Permit Application Package
Dear Mr. West,
On February 20, 2014, the Division of Reclamation, Mining and Safety (Division) received your
110(2) Hard Rock Reclamation Permit Application package for the Steel Rose 1 site, File No.
M- 2006 -031. Preliminary review of the information received determined that the following
items must be received before the Division can consider your application as being submitted and
technical review can begin:
Application Form:
1. The Division requires the company name listed throughout the application (including within
the notarization) match the company name listed with the Colorado Secretary of State (SOS).
A print out from the SOS website, with the correct company name listed, has been attached
to this letter. Please revise the applicant /operator name on Page 1 of the application form.
2. On Page 7 of the application, the applicant /operator name should match the company name
listed with the Colorado Secretary of State. In addition, the notary section on Page 7 of the
application was filled out incorrectly. A copy of Page 7 with the necessary corrections has
been attached to this letter. Please revise Page 7 accordingly.
Your application will not be considered filed until the information listed above is received and
found sufficient to begin our review. A decision date will be established 30 days from the date of
receipt of all of the requested information. Additionally, if you have already published notice you
will need to republish notice, after the Division considers the application submitted. Republication
must occur once within ten (10) days of the submittal date. We will notify you when you should
initiate republication of your notice. The final date for receiving comments is ten (10) days after
the date of publication or the next regular business day.
