ameO performed geosynthetics CQA activities in accordance with the CQA Plan. Geosynthetics CQA activities performed by AMEC included: • Observation and documentation of geomembrane deployment, trial seams, seaming, destructive testing, and non - destructive testing (pressure tests and vacuum tests); • Performing conformance testing of geomembrane; • Measuring geomembrane panel thickness and confirming adequate panel overlap; • Visually inspecting geomembrane sheet quality, sheet grinding, seam alignment, and continuity; and • Retaining archive samples of geomembrane destructive tests and trial seams. 1.3 Project Coordination AMEC CQC and CQA activities were directed by the AMEC Project Resident. The AMEC Project Resident (CQA Supervisor) for the construction of the 2011 Phase 5 VLF OSA 2nd Portion for the period January 2009 to August 2011, was Mr. Joseph D. Hickey. The Certifying Engineer and Project Manager, Mr. Jay Janney- Moore, P.E., or his representative, Ms. Kimberly Morrison, P.E., R.G., Project Sponsor, made periodic site visits to resolve technical issues and to review AMEC activities. Appendix C presents a summary of the AMEC staff on -site during the period of January 2009 through August 2011. Representatives of CC &V, Ames, Lone Star, and AMEC conducted daily, weekly, and preconstruction coordination meetings throughout the construction period. 1.3.1 Daily Construction Summary Reports AMEC prepared daily construction summary reports as well as photographic documentation (Appendix B) throughout the project to document the key elements of construction and the CQA activities that occurred. On -site AMEC CQA personnel, the Certifying Engineer and the Project Sponsor discussed construction activities on a regular basis.
