Cripple Creek & Victor Gold Mining Company <br />Cresson Project <br />Quality Assurance Monitoring and Test Results <br />Phase 4B /C Triangle Liner Certification <br />August 10, 2010 <br />AMEC personnel observed the testing of trial seam samples and documented test results. <br />AMEC observed the tensile testing of each trial seam coupon to verify that project yield <br />strengths were met during shear testing and that failure did not occur in the weld during peel <br />testing. When trial seams did not meet specifications, AMEC verified that the welding apparatus <br />was not used until two consecutive trial seams performed by the same technician /apparatus <br />combination met specifications. The trial seam test results for fusion seaming are presented in <br />Appendix H -2. Lonestar used two tensiometers during the project. Figures 2 and 3 show the <br />codes used to designate passing and failing test results. <br />6.1.3.2 Seaming Observations <br />AMEC personnel observed seams before, during and after welding. AMEC checked each seam <br />for adequate overlap prior to seaming and to verify that the seam area was clean and dry. <br />AMEC documented Lonestar personnel performing the seaming and the seaming equipment <br />used. Following the completion of seaming and routine repairs, AMEC checked the full length <br />of each seam to verify that no additional repairs were required. In the event that additional <br />repairs were needed, AMEC marked the location requiring repair on the geomembrane, <br />documented the location, and brought the repair area to the attention of Lonestar. Once <br />Lonestar made repairs, AMEC checked the repair location to verify completion. AMEC <br />inspected all seams for quality and completion. AMEC personnel conducted a final <br />walk- through to verify completion of the entire geomembrane system and to identify any <br />additional damage that may have occurred during construction operations. <br />6.1.4 Defect and Repair Observations <br />AMEC inspected geomembrane panels for damage that may have occurred during <br />manufacture, transport, deployment, or installation as discussed in Section 5. AMEC observed <br />all panels and seams for defects until Ames covered the geomembrane with DCF. <br />AMEC documented defects by assigning a unique defect identification code to each defect, and <br />marking each defect code on the geomembrane. The summaries of defects, repairs, and <br />non - destructive testing of repairs, including the approximate locations of each repair are <br />presented in Appendix H -4. <br />6.1.5 Destructive Testing <br />AMEC selected destructive test locations based on Lonestar's completion of geomembrane <br />welding, various working conditions, and on seaming observations (e.g., suspicion of a defective <br />weld).
