February 18, 2014 C- 1981- 044/Williams Fork Mines JLE <br />Pond 9A -P4 was holding a small amount of water. The embankment was stable and the primary spillway gate was <br />closed but appeared functional. <br />GENERAL MINE PLAN COMPLIANCE: <br />The site is currently in temporary cessation. <br />The No. 5a portal was observed from a facilities area across highway 13 near the No. 5 portal area. The haul road <br />to the No. 5a portal was impassable due to the snow and mud conditions. Given this, a detailed inspection of this <br />area was not possible. However no problems were noticed from the lower vantage point. <br />ROADS — Rule 4.03; Construction 4.03.1(3)/4.03.2(3), Drainage 4.03.1(4)/4.03.2(4), Surfacing and <br />Maintenance4.03.1(5) and (6)/4.03.2(5) and (6), Reclamation 4.03.1(7)/4.03.2(7): <br />Near the 7 North Angle dewatering hole on the eastern end of the site, snow had drifted over the haul road and our <br />vehicle became stuck in the snow. We were able to get the vehicle out but were not able to proceed to the No. 5a <br />portal. <br />RECLAMATION SUCCESS - Rule 4.15, Rule 3: <br />The No. 5 and the No. 9 portal areas were observed. These areas appeared stable. <br />REVEGETATION — Rule 4.15; Vegetative Cover; Timing: <br />The strip pit was partially snow covered, no problems were observed. <br />The Utah track was inaccessible at the time of the inspection. <br />SIGNS AND MARKERS — Rule 4.02: <br />The mine entrance signs contain an error. The DRMS permit number is listed on the sign as C- 081 -044. The <br />permit number should read C- 1981 -044. These signs will need to be corrected. <br />TOPSOIL — Rule 4.06; Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; <br />Redistribution 4.06.4: <br />The topsoil stockpiles were observed throughout the site.
