The map submitted for Exhibit C (General Layout with Existing Conditions) does not contain <br />a labeled permit boundary. Please revise the map to include a labeled permit boundary. <br />4. As required by Rule 6.4.3(a), please revise the Mining Plan Map to include all adjoining <br />surface owners of record. <br />5. Please revise the Mining Plan Map to include a depiction of the berm that will be constructed <br />along the Big Sandy Creek. <br />6.4.4 Exhibit D — Mining Plan <br />6. Please indicate if the conveyors and screen plant will be entirely portable. If footers or any <br />other support structures are to be constructed for the conveyors and screen plant, then the <br />dimensions of those structures must be specified and included in the financial warranty <br />calculation. <br />7. The Applicant has stated that if water is encountered it will be allowed to drain back into the <br />mined area. Please note that any stormwater which is captured by this operation must be <br />allowed to infiltrate into the ground within 72 hours. <br />8. The Mining Plan states that the deposit is up to 11 feet below grade. Please indicate the depth <br />to groundwater; this information may be based on well records or other available data for the <br />area. Also, groundwater may not be exposed without a well permit, temporary substitute <br />water supply plan and appropriate bonding. Please commit to maintaining the elevation of the <br />pit floor at a minimum of two feet above the static groundwater level. <br />9. Please indicate if the access road to the pit will need to be improved or upgraded. Any roads <br />which are upgraded or substantially improved to accommodate the mining operation must be <br />included in the affected area. <br />10. Please indicate if overburden will be sold as product or stockpiled and backfilled during <br />reclamation. If the overburden will backfilled, then please specify the location of the <br />stockpiles. <br />11. Specify the grade of the mine face as mining progresses from west to east. <br />12. The Applicant has stated that topsoil stockpiles which remain in place for longer than 6 <br />months will be seeded with a summer annual plant. Please specify the species to be planted <br />and the seeding rate. <br />13. The Applicant has stated that diesel fuel will be stored on site. Please specify the volume of <br />diesel fuel to be stored and describe the secondary containment structures which will be used. <br />Also, note the location of fuel tanks on the Mining Plan Map. <br />
