PERMIT #: M- 2012 -004
INSPECTOR'S INITIALS: ECS
INSPECTION DATE: February 5, 2014
OBSERVATIONS
This inspection was conducted by Eric Scott of DRMS as a routine monitoring inspection of the 110c North Star
Pit. Permittee representative Jack Miller was present during the inspection. The pit is located in an area that
is still being actively farmed, and the site identification sign was present near the access point to the site at
county road 138.
The disturbed area associated with the mining activity to date was outlined by a ditch visible on the attached
aerial photo. The outline of the disturbed area to date has been measured from aerial photography at
approximately 12.5 acres. The attached map shows the existing boundary defined by the ditch and disturbed
area as a yellow line.
The currently approved permit boundary as constructed from the legal description and maps submitted in the
permit application is shown on the attached map as a red line. As can be clearly seen on the attached map,
the location of the disturbed areas and access road observed during the inspection, do not correspond with
the permit boundary approved by the Division. There are currently several acres of disturbed area that are
outside of the approved permit boundary shown in red.
As stated on the first page of this report, DRMS will allow until May 2, 2014 (2 year anniversary of the 110c
permit issuance date) for the permittee to submit a complete 110c to 112c permit conversion application to
incorporate (at least) all the currently - disturbed area into a new permit boundary: If a complete amendment
has not been received by the Division by May 2, 2014, the Division will initiate enforcement action for the
disturbance of areas outside of the approved permit boundary. Enforcement will consist of a Mined Land
Reclamation Board hearing that may result in corrective actions and civil penalties if a violation is found.
Inspection Contact Address
Jack Miller
Superior Oilfield Services Co., LTD
1011 11`s Ave
Greeley, CO 80631
Enclosure
CC:
.
