PERMIT #: M- 1994 -096 <br />INSPECTOR'S INITIALS: ECS <br />INSPECTION DATE: February 5, 2014 <br />OBSERVATIONS <br />This was a routine succession of operator inspection conducted by Eric Scott of DRMS for the Glacier Rock site. <br />This permit was transferred from Pittington Construction to Glacier Rock effective January 14, 2014. Glacier <br />Rock posted a $2000 reclamation bond as part of the approved Succession of Operator (SO) process. Glacier <br />Rock is now obligated by all terms and conditions that were approved under the original permit unless and' <br />until they are modified by the new permit holder and approved by DRMS. The new permittee, Mr. Hageman, <br />met DRMS on site and drove DRMS around the site for this inspection. <br />The permit identification sign and boundary markers were present during the inspection, however more <br />significant permanent permit boundary markers should be placed prior to commencing regular operations at <br />the site. Mr. Hageman also stated his intent to fence off a 50' buffer area surrounding a large high - voltage <br />power line tower near the site entrance; and DRMS agrees that this would be an excellent idea. <br />The prior operator had completed all required earthwork and seeding at the site in preparation for final <br />release, and mining has not yet resumed at the site, therefore the existing reclamation liability at the time of <br />this inspection is minimal. The new /current reclamation bond held by DRMS will allow for the disturbance of <br />5 acres using an estimate of approximately $4000 /ac for re- sloping, topsoil replacement, and seeding. If more <br />than 5 acres of land will be disturbed at any time, DRMS should be notified so that the reclamation bond can <br />be adjusted to reflect the additional disturbance. <br />Insnection Contact Address <br />Roger Hageman <br />Glacier Rock Company <br />3501 E.
