January 29, 2014
Colorado Division of Reclamation, Mining, and Safety
1313 Sherman St. # 215
Denver, CO 80203
M- 2012 -032 Revenue Mine Annual Report
To Whom it May Concern,
Below are the comments in regards to each section of the annual report form which state "see
attached"
Part 4:
During the past year, construction efforts were continued for the underground mill including the
installation of concrete, steel and equipment. With the mill being in construction the site has not
processed any ore for the production of concentrate yet. The last date of activities on the site
was January 29, 2014, which is the date of this letter.
Part 14:
With no additional tailings or significant new volumes of waste rock being added to the surface
of the site, the areas which require reclamation are mostly historical. During the past year, the
mine has begun reclamation on the historical waste piles and will place appropriate soil on the
slopes of these waste piles. At this time the soil is not on site and cannot be brought to site
reasonably until after the winter season has passed. This is planned to occur during the
upcoming summer.
Part 20:
The permit requires quarterly water sampling results to be submitted and all of these have been
submitted already. The permit also requires the submission of SPLP tests for generated tailings;
however, no tailings have been generated in the past year as the mill is in construction and not
producing tailings yet. All other required items are still in construction and have not been able to
be tested as the site is not producing material yet.
Sincerely,
_ r
1
R illiams
ager
Star Mine Operations LLC
1675 Larimer St. Suite 820
Denver, CO 80202 RECEIVED
FEB 0 5 2014
Division of Reclamation,
Mining & Safety
