PERMIT #: M- 1987 -113
INSPECTOR'S INITIALS: PSH
INSPECTION DATE: January 30, 2014
feet in depth and is mined in layers with benched side slopes. The mined slopes appeared stable. A small
about of rainwater was observed in the pit. The mining activity will continue to the north and deeper in the
future. The mined material is processed in the adjacent Church Pit permitted by Lakewood Brick. An
agreement between Bestway and Lakewood Brick exists for material mined from both sites to be processed at
the Church Pit. A concrete recycling operation was observed north of the pit.
The entrance sign and boundary markers were observed during the inspection as required by Rule 3.1.12. The
location of the northeast and southeast boundary markers were not properly marked at the time of the
inspection. As cited as a problem on the first page, the Operator agreed to conduct a survey of the site to
accurately locate the permit boundary corners and install permanent markers. The western boundary
markers were located during the inspection. The western permit boundary is shared with the Church Pit. The
boundary markers were GPS surveyed by the Division with a Trimble Juno 3B. A map indicating the results of
the survey is attached. The Division holds a financial warranty in the amount of $550,931.00, which appears
adequate to reclaim the site based on this inspection.
Photographs taken during the inspection are attached below. A copy of the GPS map created by the Division
during the June 8, 2009 inspection is attached.
Inspection Contact Address
Mr. Mark Johnson
Bestway Concrete Company
301 Centennial Drive
Milliken, CO 80543
Enclosure — GPS Survey Maps
Cc: Tom Kaldenbach, DRMS
