P 1 f /l' ) <br />e <br />Brookfield <br />Residential <br />January 21, 2014 <br />Mr. Tony Waldron <br />Colorado Division of Reclamation, Mining and Safety <br />r. <br />1313 Sherman Street, Room 215 <br />Denver, CO 80203 <br />RE: Annual Report <br />DMG Permit Amendment No. M- 1982 -015, AM -1 <br />Job No. 3811 -DMG <br />Dear Mr. Waldron, <br />Bayshore, LLC is submitting this Annual Report letter for permit number M- 1982 -015 <br />to the Colorado Division of Reclamation, Mining and Safety (Division). This permit <br />was originally listed under Siegrist Construction Company. A permit amendment was <br />approved by the Division as Amendment 01 on September 25, 2006, and a <br />Succession of Operators Approval was granted by the Division on October 25, 2006. <br />The new operator is now Bayshore, LLC (Formerly Carma Bayshore, LLC). Recorded of <br />the corporate name change has been filed with the Division. <br />The anniversary of the original permit is December 10. We understand that an Annual <br />Report is due each year at that time. The report should address any activities that <br />have occurred within the permit area. A check for the permit's annual fee in the <br />amount of $791 is attached. No changes to the map on file are necessary as no new <br />disturbances or reclamation has occurred during this year. <br />A revised grading and reclamation plan was submitted and approved by the Division in <br />2012. Work for the final shape grading and soil enrichments where well underway in <br />September 2013. The historic floods during September breached the embankment of <br />the facility. Our team has been working with FEMA, Army Corps of Engineers, and <br />Weld County to prepare repair plans.
