Cripple Creek & Victor Gold Mining Company <br />Laboratory Liner Extension <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 14, 2014 <br />3.0 Geosynthetic Materials Installation <br />ameO <br />Geosynthetics installed for the Laboratory Liner Extension consisted of GCL and 80- <br />mil single -sided textured LLDPE geomembrane with an overlay of geotextile. The <br />BentoLiner NWL GCL and the LLDPE geomembrane were manufactured by GSE. <br />Geosynthetic installation included deployment and seaming of GCL. Seaming of GCL <br />consisted of overlapping the GCL material with a bead of bentonite powder between <br />the overlap. Deployment, seaming, non - destructive, and destructive testing of 80 -mil <br />single -sided texture LLDPE geomembrane was performed at the required intervals. <br />Lone Star Lining Company (Lone Star) installed approximately 1,204 square feet of <br />both GCL and geomembrane. <br />GLC was placed on compacted and approved subgrade followed by the <br />geomembrane. The geomembrane was secured in one of two ways for this project. <br />Where the liner was tied to the existing concrete slab; it was held in place using a <br />stainless steel batten strip anchored by 3/8- inch - stainless steel expansion bolts. For <br />the new slab, the liner was extrusion welded to a HDPE "T- Lock" embed placed by <br />Ames into the new concrete slab. As- builts of the 80 -mil single -sided textured LLDPE <br />geomembrane have been included in Appendix A. <br />3.1 CQA/CQC Monitoring for Geosynthetics Installation <br />AMEC provided CQA monitoring of geosynthetics used for the Laboratory Liner <br />Extension. AMEC performed geosynthetics CQA activities in accordance with the <br />CQA Plan. Geosynthetics CQA activities performed include: <br />• Reviewed the product certifications of all geosynthetics installed to ensure they met <br />project specifications <br />• Observed and documented GCL, geomembrane, and geotextile deployment to <br />verify it was performed in accordance with project specifications and design <br />• Observation and documentation of GLC seaming performed in accordance with <br />manufactures recommendations <br />• Performed and reviewed conformance testing of geomembrane and GCL <br />• Observation of trial seam testing, destructive testing, and non - destructive testing <br />for geomembrane welds <br />• Measured geomembrane panel thickness to confirm it met project specifications <br />• Confirmed adequate GCL, geomembrane, and geotextile panel overlap <br />• Visually inspected GCL, geomembrane, and geotextile sheet quality, seam <br />alignment, and continuity <br />• Retained archive samples of geomembrane destructive tests and trial seams <br />sAprojects \1125m -cc &v lab liner extension\h2 - design \lab liner extension \roc report\lab liner roc report.docx 6 <br />
