The following problems were to be resolved on or before <br />September 5 and November 4, 2013 respectively. The description and corrective action for the problems <br />are listed below. <br />Please resolve the corrective actions and provide evidence of resolution on or before April 14, 2014. <br />Please note if the corrective actions are not properly addressed and evidence provided to the Division by <br />the date provided above, the listed problems could be escalated to the level of possible violations and <br />would be referred to the Mined Land Reclamation Board for further action. <br />PROBLEM /POSSIBLE VIOLATION: Problem: Rule 1.13.5 states, if an Operator temporarily ceases <br />production of the mining operation for one hundred eighty (180) days or more, the Operator <br />must file a Notice of Temporary Cessation in writing to the Office. <br />CORRECTIVE ACTIONS: The Division recommends the Operator file for Temporary Cessation or <br />resume mining activities at the site. <br />PROBLEM /POSSIBLE VIOLATION: Problem: The permit boundary markers are missing or <br />incorrectly placed. This is a problem for failure to maintain boundary markers around the <br />permit area as required by Section 3.1.12(2) of the Rule. <br />CORRECTIVE ACTIONS: The Operator shall conduct a survey and replace the boundary markers <br />in the correct location(s).
