The following items are <br />incomplete with the application submission: <br />1. Prospective Operator name listed as Lewis Fillmore, signed R. Lewis Fillmore and Financial Warranty listed as <br />R. Lewis Fillmore. Please clarify that Operator name should be R. Lewis Fillmore, an individual. <br />2. Demonstration of Legal Right to Enter not provided, per instructions. See attached. <br />3. Structure Agreements not provided per requirements of Construction Material Rules 6.3.12 and 6.4.19.
