Section 2.03.12 <br />2.03.9 Personal Injury and Property Damage Insurance Information <br />Oxbow Mining, LLC will provide the Division with the required certificate of insurance on an annual <br />basis and retain a copy in its files. <br />2.03.10 Identification of Other Licenses and Permits <br />(1) Permit to Store and Use Explosives No. 370 from the Colorado Division of Mines. Rendered <br />unnecessary with BATF changes after 9/11. <br />(2) Colorado Discharge Permit System, CDPS Permit No. CO-0000132. Issued by the Colorado <br />Department of Public Health and Environment, Water Quality Control Division, 4300 <br />Cherry Creek Drive So., Denver Co 80222-1530. Effective July 1, 1992, renewed October <br />1996, and again to be renewed October 2001, to include stormwater management provisions <br />previously addressed under Stormwater Permit COR-040000. See Exhibit 2.03.E4 for copies <br />of permits. <br />Colorado Discharge Permit System, COR-040000, Facility No. COR-040084, CDPS General <br />Permit for stormwater discharges associated with mining activity. Effective November 5, <br />1992. Incorporated into CDPS Permit CO-000132. <br />(3) Colorado Department of Public Health and Environment, Air Pollution Control Division, <br />4300 Cherry Creek Drive So., Denver, CO 80222-1530. See Exhibit 2.03.E4 for copies of <br />permits. <br />Permit No. 98GU0812, issued August 10, 2001, for Sanborn Creek and Elk Creek Mines <br />located immediately north of Somerset, Gunnison County, Colorado and including <br />underground coal mines, coal processing facility, associated equipment, coal handling, coal <br />stockpiling, and hauling. <br />Permit No. 98GU0833L, issued March 11, 1999, expires December 31, 2001, for land <br />development project for development of the portal and surface facilities for the Elk Creek <br />Mine, located north of Highway 133, about ten miles northeast of Paonia, Gunnison County, <br />Colorado and including portal area grading and associated construction activities. <br />(4) Gunnison County, 200 E. Virginia, Gunnison, Colorado 81230. Gunnison County Land Use <br />Change Permits. See Exhibit 2.03.E4 for copies of permits. <br />LUC No. 1990-51 for the Sanborn Creek Mine and surface facilities. <br />LUC No. 1991-37 for the East Yard and Elk Creek stockpiles. <br />LUC No. 1991-40 for the reclassification of a designated access road to a haul road for the <br />operation of the Elk Creek facilities with coal hauled from other mines in the area. <br />LUC No. 1993-58 to allow projection from the Sanborn Creek mine to increase to about 1.3 <br />milllion tons per year. <br />LUC No. 1993-60 for installation of a second ventilation fan for the Sanborn Creek Mine. <br />Notification letter, dated August 5, 1996, from Staff Planner that relocation/ construction of <br />new shop, office, and warehouse facilities was covered under existing plan approvals and <br />would not require separate County authorization. <br />LUC No. 1996-055, approved March 18, 1997, to construct new West Valley Fill coal refuse <br />disposal facility, haul road, and sedimentation pond. <br />RN-05 2.03-17 Rev.
