This i <br />Curtis <br />PERMIT #: M -1990 -047 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: March 14, 2013 <br />OBSERVATIONS <br />m was conducted as part of the regular monitoring program. The Operator was represented by <br />who was present for the inspection. This is a 110c gravel mine operated by Fremont County. <br />The Qu4gmire Creek Pit is located approximately 12 miles north of Cotopaxi on the west side of Fremont Co Rd <br />12 in Fr mont Co. <br />A permi sign had been displayed at the site entrance (see Photo 1). Two topsoil stockpiles were observed to be <br />stable (s ,-e Photos 2 and 3) and also identified with a sign. Corners of the pit area were also marked (see Photo <br />4). The upper highwall appears to be cut at about a 1H:1V slope and daylights near the permit boundary (see <br />Photo 5). <br />The pit was not active at the time of the inspection. No equipment was observed on site. The site drains <br />towards FC Rd 12 and no standing water was observed in the pit. <br />As the pit is adjacent to Co Rd 12, there is essentially no haul road. An unscreened material stockpile was <br />observed on the south end of the pit floor (see Photo 6). No noxious weeds were observed. <br />File Review: <br />• The total permitted area is 6.4 acres. <br />• The previous inspection was conducted on 6/17/2011, no problems were cited. <br />• This is a county pit, therefore no financial warranty exists. <br />• Annual fees and reports are up to date through 2013. The 2013 Annual Report indicates 3.54 acres have <br />been disturbed. <br />• The map submitted with the 2013 Annual Report is not the latest map included as Exhibit E of the <br />Amendment 1 application (see attached) <br />• Review of satellite images and the attached mine plan map indicate the extent of mining on the west side <br />of the permit is very close to the permit boundary. This inspector estimated 3 to 3.5 acres have been <br />disturbed based on review of 9/24/2013 satellite images. <br />• The post -mine land use is wildlife habitat. <br />Current Stipulations: <br />None. <br />Summary & Recommendations: <br />1. The Operator is very near the western permit boundary. The site should be surveyed to ensure mining <br />activities are within the permit boundary. <br />2. Future annual reports should include a map with an updated /current permit boundary, such as that on the <br />attached map. <br />Page 2 of 4 <br />
