EXHIBIT L Reclamation Costs
Reclamation costs have not been calculated because Phillips County is a local government.
EXHIBIT M Other Permits and Licenses
A zoning permit from Phillips County has been obtained.
An Air Pollution permit will not be needed since we will remove less than 70,000 tons per year
from this site.
This is an open pit operation and ground water will not be exposed, so a well permit and
temporary supply pan is unnecessary.
A storm water management plan will be prepared for this pit if there is a potential for surface
water leaving the site.
A dredge fill permit (404) is not required because waters of the United States will not be
impacted. Notification will be given to the Corps of Engineers.
Notification of the proposed mining was sent to the State Historic Preservation Officer. Their
search showed no significant historic artifacts at the site.
No permits are required from the Bureau of Land Management or US Forest Service.
No additional permits or licenses are required.
EXHIBIT N Source of Legal Right to Enter
Greg and Denise Smith are the owners of the property in question. A copy of permission to
mine granted by Mr. and Mrs. Smith is attached.
EXHIBIT 0 Owners of Record of Affected Land (Surface Area) and Owners of Substance to be
Mined
Owners of Severed Mineral Interest
Phillips County
c/o Phillips County Commissioners
221 S Interocean Ave
Holyoke, CO 80734
Owners of Affected Land
Phillips County
c/o Phillips County Commissioners
221 S Interocean Ave
Holyoke, CO 80734
Phillips County Landfill Page 11
