PERMIT #: M- 2009 -006 <br />INSPECTOR'S INITIALS: ECS <br />INSPECTION DATE: December 11, 2013 <br />OBSERVATIONS <br />This was a routine monitoring inspection conducted by Eric Scott of DRMS for the Meadow Hollow Quarry <br />site. DRMS met with permittee Wayne Dorband and site operator Matt Sprague on site at approximately 1200. <br />The site entrance is located off of County Road 31 (Meadow Hollow Road) on the west side of the road <br />approximately 3/4 mile after the pavement ends, near a pre -law quarry site, and is accessed via a private gate. <br />The required site identification sign was present. <br />As noted on the first page of this inspection report, the permit boundary markers were either missing or unable <br />to be located. The boundary markers will need to be replaced in the proper locations, and proof submitted to <br />DRMS that this has been completed by the Corrective Action Due Date listed on the first page of this report <br />(1/31/14). <br />Mining activity appears to be proceeding according to the approved mining plan and approximately 5 acres has <br />been disturbed at this time. <br />As in the pre -op inspection, it was noted that the majority of the site is steeply sloping to the east and runoff <br />from a strong storm event may leave the site if some runoff control measures are not implemented. Run -on <br />control may help minimize potential site drainage issues as well, as there is currently a natural runoff pathway <br />through the site that could carry water during a major storm event.
