Signs and Markers (Rule 4.02): The mine identification sign at the entrance to the site is intact <br />and contains all necessary information in accordance with 4.02.2. <br />The perimeter is marked with either fencing or white markers where fencing is absent (4.02.3). <br />All observed topsand stockpiles are marked in accordance with 4.02.7; however, one marker on a <br />topsand stockpile on the north end of the permit area near Pond 2 has fallen and should be <br />replaced. <br />No disturbances to areas outside the permitted area were noted. <br />The following items were verified on file at the Keenesburg Mine office during the records <br />check: <br />• DRMS permit: (exp 10/14/16) <br />• Permit Application Package with revisions <br />• Most recent findings documents (RN -6) <br />• Liability insurance (exp. 11/1/13 but has been renewed — Ben will email copy to Jared) <br />• Bonding documents (most recent rider 3/14/12) <br />• Bond release documents (SL -6 2/27/12) <br />• Air emissions permit (exp. 2016) <br />• County special use permit <br />• Rill and gully survey <br />• Annual Reclamation Report (2012) <br />• Annual Hydrology Report including groundwater monitoring data (2012) <br />• Midterm review documents (2009) <br />• Over 3 year's DRMS / OSM inspection reports (through 10/2013) <br />• Pond certification as- builts for Pond 2 and Dugout <br />• Annual pond certifications: quarterly pond inspections are conducted by a P.E. <br />• Haul & access road certifications (1984) <br />• NPDES permit termination documents <br />• SPCC plan <br />• 3rd 2013 Quarterly pond inspections (9/19/13) <br />Maintenance items identified during inspection: <br />• Please replace the topsand marker on the northern stockpile near Pond 2. <br />• Please email the renewed liability insurance certificate to Jared Ebert. <br />No compliance issues were noted and no enforcement actions were taken as a result of this <br />inspection. <br />W <br />
