• NPDES permit (administrative extension 6/1/2013) <br />• NPDES records (11/4/2013) <br />• Stormwater management plan (12/2009) <br />• SPCC plan <br />• 3`d Quarterly pond inspections <br />• Annual hydrology report (2012) <br />o Groundwater monitoring <br />• Surface water monitoring <br />• Spring and seep monitoring <br />• Mine water discharge monitoring (mine water is pumped to B -seam) <br />• Well permits <br />• ATFE explosives permit <br />• Applicant Violator System ownership and control info was verified <br />Maintenance items identified during inspection: <br />• The culvert routing Pond E discharge under the railroad toward the North Fork of the <br />Gunnison is filling with debris on both ends, although the interior of the culvert appears <br />clear. This is likely due to railroad maintenance activities, but is within the permit <br />boundary and should be cleaned out. <br />• Please ensure proper sediment control structures are in place in the clear water diversion <br />adjacent to the Elk Creek Haul Road south of the rock dust tanks and north of Pond D. <br />Dense vegetation could be concealing old hay bales, and sediment is accumulating in the <br />area just above the discharge culvert. <br />• A sediment trap on the north side of the West Valley Haul Road (north of the cemetery) <br />has been damaged and requires maintenance. <br />• The Forest Service is interested in retaining the clearing at the sealed utility borehole near <br />the reclaimed Hubbard fan as a stock trailer parking area. This would require a permit <br />revision to change the post mining land use. <br />• Please ensure retention of raw / potable water tanks and treatment facility is part of the <br />approved reclamation plan. <br />No compliance issues were noted and no enforcement actions were taken as a result of this <br />inspection. <br />
