PERMIT #: M- 2012 -032
INSPECTOR'S INITIALS: RCO
INSPECTION DATE: November 1, 2013
specs), so it is doubtful that they will be installed before next spring.
The old mine pond is empty, and its floor is fairly rough. The approved mine plan is for this depression to be
the first area where waste rock and new dried tails will be placed. Before this begins the floor will be graded,
and the contaminants and debris items removed for proper disposal.
Other site observations included: the old wood - cribbed avalanche deflection structure is gone, and potable
water storage tanks are placed there. The snow roof structure over the lunch and battery charging area has
not been installed yet (since mill construction is the present priority).
There are several areas where the approved "half culvert" type stormwater conveyance ditches have been
installed, but too much drifted snow was covering them to adequately observe them. More of these
structures are planned under TR -1 and will be installed in the coming year.
The operator's quarterly surface and groundwater monitoring sampling program continues, with analyses
forwarded to the Division in a timely fashion. No monitoring wells have been installed yet below the waste
rock dumps along Sneffels Creek. These will be in place next year.
The recent waste rock storage area east of the site access bridge still has a steep outslope, since there is still
some mill equipment stored on its upper surface. After the equipment is removed the waste rock slope will
be reduced to 3:1.
The bond has been fully recalculated in association with the TR -1, and the bond amount is considered
adequate. No further items were observed or discussed, and no problems were noted.
For questions related to this report, please contact this inspector at the Division's Durango Field Office:
DRMS — Durango Field Office
691 CR 233, Room A -2
Durango, CO 81301
Telephone 970 - 247 -5193
Inspection Contact Address
Rory Williams
Star Mine Operations, LLC
1675 Larimer Street
Denver, CO 80202
EC: John Trujillo, Star Mine Operations, Ouray
Inspection photos are on the following pages.
N ou. 2 of 6
