The following items are <br />incomplete: <br />1. The Application form is incomplete, page 7, Due Diligence acknowledgement and agreement. <br />2. The Application fee of $144.00 not received. <br />3. Option 2 indicated to provide current Financial Warranty in the amount of $295,172.00 not provided with <br />Affidavit of Authority for officer signing Financial Warranty. <br />4. Demonstration of Legal Right to Enter not provided. <br />5. Structure Agreements not provided. <br />In order to complete the acceptance of this application, please return the documents to the State at the above address by <br />January 18, 2014.
