This report was prepared to comply with conditions set forth in <br />Attachment B of the Bureau of Land Management's Decision Record, Finding of No Significant Impact, and <br />Final Environmental Assessment for the Whirlwind Mine Uranium Mining Project, September 2008. <br />Energy Fuels employee records show that a Mine site inspection, which included recording water level <br />measurements and required field parameters, was conducted on August 14, 2013. However, due to recent <br />employee turnover, the field notes containing the required measurements were lost. Energy Fuels has <br />addressed this issue and all field notes will be sent to Energy Fuels corporate office as soon as practicable, or <br />within a week of the required site inspection. The fourth quarter site inspection, required field parameter <br />measurements, and water level measurements will be conducted in November 2013.
