Preliminary review of the information received determined that the following items must be <br />received before the Division can consider your application as being submitted and technical review can <br />begin: <br />Application Form <br />1. The Applicant did not provide the name of the owner of subsurface rights of affected land for item <br />No. 6 on Page 2 of the Application Form. Please submit a revised page with this information <br />included. <br />2. The Applicant did not provide the name of the owner of the surface of affected land for item No. 7 on <br />Page 2 of the Application Form. Please submit a revised page with this information included. <br />3. The Applicant did not specify the quarter section or the quarter /quarter section for the location of the <br />mine site on item No. 9 on Page 2 of the Application Form. Please submit a revised page with this <br />information included. <br />4. The Applicant did not include contact information for the Inspection Contact for item No. 11 on Page <br />3 of the Application Form. Please submit a revised page with this information included. <br />6.4.16 Exhibit P — Municipalities Within Two Miles <br />a ' <br />5. The Applicant has listed the Livermore Public Works Department as a municipality located within <br />two miles of the proposed mining operation. The contact address which was provided for the <br />municipality is an out of state address. Please confirm that the address is correct and confirm that the <br />Livermore Public Works Department is an actual municipality.
