Liberty Mining, LLC <br />DRMS 110 -d Permit Application <br />October 2013 <br />EXHIBIT F OTHER PERMITS AND LICENSES REQUIRED <br />F (1) State of Colorado Department of Public Health and Environment (CDPHE) <br />Water Quality Control Division Stormwater Discharge Permit <br />As the disturbance is greater than 1 acre, a State of Colorado Construction Stormwater <br />Management Plan will be prepared, a construction Notice of Intent filed with CDPHE, and <br />proper BMP's established and maintained. Upon completion and reclamation, a Notice of <br />Termination will be submitted. <br />F (2) CDPHE Air Pollution Control Division Fugitive Dust APEN <br />The Air Pollution Emission Notice (APEN) will be applied for once the DRMS 110 -d permit is <br />under review. Once the APEN has been approved, it will be submitted to DRMS and all other <br />interested parties within 45 days of receipt. <br />F (3) Federal BATF &E Explosives Permit <br />Reliance Resources, LLC has been issued a Federal BATF &E Explosives License Number is 9- <br />UT- 019 -33 -1 F00371, valid through June 1, 2014. Reliance Resources, LLC, is the sole <br />shareholder and parent company of Liberty Mining, LLC. <br />F (4) MSHA Mine Identification <br />The Mine Safety and Health Administration assigned identification number for the Liberty Mine <br />is 05- 04819. <br />F(5) U.S. Bureau of Land Management Plan of Operation <br />This DRMS permit application will be submitted simultaneously to the US Bureau of Land <br />Management Grand Junction, Colorado office to serve as a Mining Plan of Operation. <br />Correspondence has already begun between representatives of Liberty Mining, LLC, and the <br />BLM Grand Junction office. Once given approval from the BLM, a copy of the approval will be <br />submitted to DRMS within 30 days of receipt. <br />F (6) Mesa County Conditional Use Permit <br />This DRMS permit application will be submitted simultaneously to the Mesa County Planning <br />Commission to serve as an application for a Conditional Use Permit. Correspondence has <br />already begun between representatives of Liberty Mining, LLC, and Mesa County.
