PERMIT #: M- 1978 -249 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: October 24, 2013 <br />recent lease agreement with the landowner, MMM Partnership, which expired in September 2013. The <br />Division does not have a copy of this lease agreement. As cited as a problem requiring corrective action by the <br />Operator on the first page, the Operator must submit a source of Legal Right to Enter as required by Rule <br />6.4.14 within 60 days. <br />Mining Plan <br />The site was inactive during the inspection. The current mining disturbance is located in the southeast section <br />of the site. A ten (10) feet highwall was observed with a stripped area and a topsoil stockpile above the <br />highwall. Product stockpiles were observed on the pit floor. A second excavation area was observed <br />northwest of the highwall. Stockpiles of imported material were observed in the southwest area of the site. <br />Reclamation <br />The approved post- mining land use for the site is rangeland. The approved reclamation plan requires the <br />Operator to regrade the slopes to 3H:1V with six (6) inches of topsoil spread over the regraded areas. <br />Reclamation activities have not begun at the site; however the southwest portion of the site has been <br />naturally revegetated. <br />Photographs taken during the inspection are attached. <br />Page 3 of 5 <br />
