ENVIRONMENT, INC. <br />OCTOBER 257 201 3 <br />PAGE 2 <br />nearest shot hole and the nearest house, intersects the property boundary between the mine <br />site and the adjacent residence. <br />Shots will be designed to have a maximum peak particle velocity (MPPV) no greater than 1.0 <br />inch/second at a point between the closest shot and the nearest house at a point where a <br />straight line from the hole to the house intersects the property line. <br />The Division requires all Operators using explosives to complete a blasting report for each <br />shot. The report must be retained by the Operator for at least 3 years and be available for <br />inspection by the Division on demand. The record shall contain the following data - <br />a) Location, date and time of blast; <br />b) Name, signature, and license number of blaster -in- charge; <br />c) Identification, direction and distance in feat, from t ie nearest blast hole to the nearest <br />potentially affected structure, such as any dwelling, school, church, or community or <br />institutional building either: <br />i. Not located in permit area; or <br />ii. Not owned nor leased by the person who conducts the mining operations <br />d) Weather conditions, including temperature, wind direction, and approximate velocity; <br />e) Type of material blasted; <br />f) Sketches of the blast pattern including number of holes, burden, spacing, and delay pattern <br />Sketches shall also show decking, if holes are decked to achieve different delay times within a <br />hole; <br />g) Diameter and depth of holes; <br />h) Types of explosives used; <br />i) Total weight of explosives used per hole and maximum weight of explosives per 8 millisecond <br />period; <br />j) Initiation system; <br />k) Type and length of stemming; <br />1) Mats or other protections used; <br />M) Type of delay detonator and delay periods used; <br />n) Number of persons in the blasting crew; and <br />o) Seismographic records, including: <br />i. Type of instrument, sensitivity, and the calibration signal of the gain setting or certification <br />an annual calibration; <br />ii. Exact location of instrument and the date, time and its distance from the blast; <br />iii. Name of the p,-:,r-.on taking the reading; <br />iv. Name of the person and firm analyzing the seismographic record; <br />v. The vibration level recorded. <br />The operator understands this requirement and will prepare a report on each blasting event <br />that contains the information listed in this adequacy question. The records will be retained <br />and made available for inspection upon request. <br />6. Please specify how many blasting events will occur on a daily basis <br />Typically one shot per day is planned. <br />The drill hole profile included in the blasting plan shows explosives will be loaded into a 3 inch <br />diameter drill hole that is 12 ft. deep. The dimensions of the drill hole will accommodate 30 lbs. of <br />explosives (ANFO). Please specify how a drill hole with the above referenced dimensions will be <br />loaded with 60 lbs. of explosives; provide a separate drill hole profile for blasts requiring 60 lbs. of <br />explosives, if necessary. <br />
